I have a downstream server at a remote location that has a download
status of updating files 7, downloaded 21.02 mb of downloaded 21.02
mb. This never changes. If I force a new sync, it says it's
downloading the files, and then reports the same download status. I'm
not using a proxy.
Any ideas?

Review the ApplicationEventLog of that downstream server to find any log
entries reporting why the downloads have stopped/failed.

Also, use the BITSADMIN 2.0 utility to identify the specific file(s) that
are still in the download queue, and reconcile that with the "File Status"
of each of the updates that were added in the most recent synchronization
event(s). The question goes to determining whether the update are not
actually downloaded, or simply have not been reported (from BITS) to the
WSUS server as downloaded.
